The EPR form was designed to provide the Governor's Office (GO), CalHR, and the State Controller’s Office (SCO) with the information needed to process the paperwork associated with the following:
- an exempt appointment;
- establishment of an exempt classification;
- abolishment of an exempt classification;
- conversion of a civil service position (GC 12010.6); or
- salary changes to exempt classifications.
Departments must submit a complete EPR package that consists of the EPR form, justification memo, duty statement, and organizational chart.

Exempt Salary Increase Requests
Departments are not authorized to provide any salary increases to exempt appointees, including salary increases within the current range of the exempt classification. Salary increase requests require an EPR package and approval from Agency (if the department reports to an Agency), the Governor’s Office, and the Department of Human Resources (CalHR). The complete package should be forwarded to the Governor’s Office and the EPR inbox, simultaneously.
